The landscape of cancer care in America has shifted from a few large academic centers to the community setting. Improvements in cancer outcomes achieved over the past 3 decades have also resulted in increased costs for cancer care, a burden that is increasingly being shouldered by patients and their physicians. Community oncologists can help reverse this trend by developing ways to link reimbursement with actual costs, especially for their own time, introducing greater efficiencies in their practice through increased use of technology, and raising their profile on the local and national scene by educating the public through increased and more effective contacts with the media.
