Community oncologists can help reverse this trend by developing ways to link reimbursement with actual costs, introducing greater efficiencies in their practice, and raising their profile by educating the public through more effective use of the media.

Dr. Arena and his colleagues hope the ACORN model could serve as a prototype for other community oncology practices that lack the staff or otherwise hesitate to participate in clinical trials.

This case of a 43-year-old man with a rare metastatic paraganglioma illustrates the myriad issues and challenges faced by physician, staff, patient, family, consultants, lawyers, and insurers in developing and implementing a complex treatment plan. The methods used to achieve a successful outcome for this patient are discussed, plus suggested newer approaches that might improve care for patients with this and other rare cancers.
